Conditions:
-Cancer
-PTSD
-Crohn’s
-Glaucoma
-Epilepsy
-Positive for HIV
-AIDS
-ALS
-Parkinson’s
-Terminal condition diagnosed by a physician
-Chronic non-malignant pain
-Other debilitating medical conditions in the same class

1. Come in for initial evaluation / certification $150.
2. Fill out the forms online with the state and pay $75 registration fee.
3. Get issued your Medical ma*****na card and purchase medical cannabis at a dispensary.

We will work with you to get you qualified. If you do not qualify, you will be refunded.
