Slipping Rib Syndrome! 🫁🫁🫁😥

Treatment options:

🩺 1. CONSERVATIVE MANAGEMENT

Usually the first line of treatment.

A. Activity Modification

Avoid movements that trigger pain (e.g., twisting, bending, lifting).

Bracing or taping may reduce rib motion temporarily.

B. Medications

NSAIDs (ibuprofen, naproxen) for pain and inflammation.

Short course of muscle relaxants if spasms are present.

Lidocaine Patches

C. Physical Therapy

Focused on core stabilization and postural correction.

Gentle manual therapy or rib mobilization by an experienced therapist.

Breathing exercises to reduce thoracic strain.

D. Heat or Ice

Helps reduce localized pain and inflammation.

💉 2.INTERVENTIONAL PAIN MANAGEMENT OPTIONS

Useful for both diagnosis and symptom control.

A. Local Anesthetic + Steroid Injection

Injected at the costochondral or interchondral junction.

Provides temporary or sometimes long-lasting relief.

Can confirm diagnosis if pain resolves after injection.

B. Intercostal Nerve Block

Targets the intercostal nerve running below the affected rib.

Can provide several weeks to months of pain relief.

C. Cryoneurolysis (Cryoablation) or Radiofrequency Ablation

Destroys or disables the affected intercostal nerve for longer-term relief (months to years).

Increasingly used for chronic slipping rib pain when injections are effective but short-lived.

Ultrasound-guided cryoneurolysis is often preferred for precision and safety.

🧠 3. SURGICAL MANAGEMENT

Considered when conservative and interventional options fail.

A. Cartilage Resection (Costal Cartilaginous Excision)

The mobile portion of the rib cartilage (often 8th–10th rib) is removed.

Goal: eliminate the hypermobile segment that irritates the intercostal nerve.

Reported success rates: 70–90% with significant pain reduction.

B. Rib Stabilization / Fixation

Newer techniques use bioabsorbable plates or suturing to stabilize the ribs.

May be used alone or with cartilage resection.

C. Thoracoscopic or Open Approach

Thoracoscopic (minimally invasive) techniques are emerging at specialized centers.

⚖️ 4. Emerging and Adjunct Options

Ultrasound-guided diagnosis: Helps visualize rib motion and confirm SRS.

Prolotherapy or PRP injections: Experimental; aims to stabilize lax ligaments.

Multidisciplinary pain programs if chronic pain persists.

🚨 When to Consider Surgery

Persistent pain >6 months despite conservative measures.

Repeatedly positive “hooking maneuver” (pain reproduced by lifting rib edge).

Clear imaging or dynamic ultrasound evidence of rib hypermobility.

Pain significantly limiting daily function.
