We're looking for an RN Care Coordinator!
This role will substitute the full time coordinator and sometimes come in to supplement, no fixed days. The RN Care Coordinator functions as a part of a multidisciplinary team, including physicians, social workers, discharge planning assistants and payers. This job ensures that patient’s progress in the acute episode of care through post discharge is quality driven while being efficient, cost effective and safe. This job will also be patient focused and outcome oriented.

It's a beautiful day for not just one, but three Heart Walks! The team at VCU Health Tappahannock Hospital showed their support for the ❤️ American Heart Association ❤️ wearing red tutus and walking a course around the hospital campus. They also hosted a chili cook off fundraiser to support their Heart Walk teams.

We're celebrating ! Our team in Tappahannock it top notch...but don't just take our word for it. We recently celebrated a successful DNV reaccreditation for the hospital as well as our ISO 9001 certification. Several functions of the hospital were celebrated by the surveyors with the most notable being environmental services. The lead surveyor called a all hands EVS staff meeting to recognize the team for a job well done! 🎉
