We are proud partners with the Veteran Affairs Community Care Network, providing compassionate, personalized home healthcare, including AFC, GAFC, and Private Nursing. At Rivers of Hope, we are dedicated to enhancing community life for our members and their families through exceptional home healthcare solutions. We specialize in personalized care programs, including Home Healthcare, Veteran Care, Adult Family Care (AFC), and Group Adult Foster Care (GAFC), Private Care, each designed to support the unique needs of our clients in the comfort of their homes or in loving, family-like settings. Our Adult Family/Foster Care programs provide care in secure private residences, offering support for the elderly, adults, and adolescents with disabilities or chronic illnesses. Many of our AFC clients are lovingly cared for by a parent, child, or other family members who qualify as caregivers. For our veterans, we deliver specialized care that honors their service and addresses their specific health challenges. Financial and Health Support:
As a MassHealth-funded program, our AFC provides caregivers with a tax-free daily stipend, ensuring financial relief while they provide critical care. Our commitment extends beyond mere assistance—our staff becomes an extension of your family. Each client and caregiver benefits from a dedicated nurse and care manager, ensuring regular visits and consistent support. Our multidisciplinary team is always on hand to provide medical training, answer health questions, and help navigate the complexities of living with disabilities. Extensive Services and Support:
We offer a broad range of services to ensure comprehensive care and support:

Michelle and I were deeply saddened to hear about the passing of a true giant, the Reverend Jesse Jackson.

For more than 60 years, Reverend Jackson helped lead some of the most significant movements for change in human history. From organizing boycotts and sit-ins, to registering millions of voters, to advocating for freedom and democracy around the world, he was relentless in his belief that we are all children of God, deserving of dignity and respect.

Reverend Jackson also created opportunities for generations of African Americans and inspired countless more, including us. Michelle got her first glimpse of political organizing at the Jacksons’ kitchen table when she was a teenager. And in his two historic runs for president, he laid the foundation for my own campaign to the highest office of the land.

Michelle and I will always be grateful for Jesse's lifetime of service, and the friendship our families share. We stood on his shoulders. We send our deepest condolences to the Jackson family and everyone in Chicago and beyond who knew and loved him.

Mission Statement:

Rivers of Hope is a family oriented, community based organization dedicated to providing the highest quality of services and advocacy for children and adults with intellectual, developmental, and physical disabilities. Our mission is to empower patients to self-advocate for their rights to autonomy. An extension of our mission is to provide financial, educational, and instructional support to family members who will be assisting the individuals. Utilizing a person-centered approach of care maximizes our members independence, rights to beneficence and non-maleficence, fosters inclusion within families, and builds community connections. Rivers of Hope is committed to patient and family advocacy in order to improve the quality of life for individuals and their families.
