Last week, our Activities and Therapy teams set out to create a special day for our residents! Together, they organized a Fall Color Tour, giving residents the chance to get out of the center and enjoy the beautiful autumn colors. 🍁

✨ Many may not know, but yesterday marked the beginning of Case Management Appreciation Week! ✨

We’re incredibly grateful for the amazing Case Management teams at our local hospitals who work tirelessly to help place patients in the best care settings possible. 🤝🏥 Their partnership is essential, and we truly couldn’t do what we do without them!

To show our appreciation, we delivered a Hot Chocolate Bar 🍫☕ — a small gesture to say a big THANK YOU for their compassion, dedication, and unwavering commitment to the community. 💙
