As we say goodbye to 2025 and welcome a fresh new year, we want to thank our patients, families, staff, providers, and community partners for your incredible support. Because of you, this past year was filled with growth, celebrations, and milestones!

✨ Top 5 PCMH Highlights from 2025:
1️⃣ Celebrated 80 years of caring for our community and 10 years at our current campus 🎈
2️⃣ Continued progress on our new Main Street Clinic, bringing expanded Women’s Health and Dermatology services closer to home
3️⃣ Successfully launched new youth apprenticeship and workforce development partnerships with local schools
4️⃣ Expanded access to services through our Outpatient Clinic with the addition of 3 new service lines, Tele-Endocrinology, Tele-Rheumatology, and Bone Health.
5️⃣ Hosted multiple community events and outreach activities that brought our community together in meaningful ways

🎄✨ Holiday cheer filled PCMH this past Friday as we hosted our Holiday Treat Party along with Pictures with Santa 🎅 and The Grinch 💚!

From sweet treats to lots of smiles and laughter, it was a wonderful day spent enjoying the season with our amazing colleagues and the community we’re proud to serve. We’re incredibly thankful for our PCMH family and for everyone who joined us to celebrate together.
