As we close out another year in Telluride, we’re reflecting on the moments that made a difference. The new patients we welcomed, the preventive care visits that helped catch concerns early, the families we supported, and the continued trust this community places in our team. * Higher Risk Patients: If you have a BRCA1/BRCA2 gene mutation, received chest radiation before age 30, or have certain hereditary syndromes, your provider may recommend a more intensive schedule—sometimes alternating mammograms with breast MRIs every six months. Genetic testing is something that you can discuss with your provider if you have a strong family history of certain cancers.

* Follow-Up After an Abnormal Result: If a mammogram shows something unusual, your provider may recommend additional imaging such as a diagnostic mammogram or ultrasound. Sometimes the plan is a short-term follow-up in 6 months, while other times you’ll return to your regular annual schedule.

At Telluride Regional Medical Center, we’ll review your records to see when your last mammogram was completed, and if nothing is on file, we’ll ask you directly to make sure you get the right care.

As cooler weather sets in, cases of upper respiratory infections (URIs) inevitably rise. During this time, many parents and caregivers face a familiar dilemma: Should I bring my child in for testing, or wait it out at home?

When to Seek Emergency Care

* A fever of 104°F (40.0°C) or higher which is unresponsive to Tylenol or Ibuprofen
* Unresponsiveness or extreme lethargy
* If child appears to be struggling to breathe
* Persistent vomiting
* Inability to drink fluids (Infant: 3-4 hours / Toddler & above: 6-8 hours)
* No urine output (Infant: 4-6 hours / Toddler & above: 6-12 hours)
* Severe sore throat or swollen glands that make it difficult to swallow or breathe

When a Primary Care Visit Is Appropriate

If your child is alert, able to drink fluids, and their symptoms do not match those listed above, it’s reasonable to manage symptoms at home or to schedule an appointment with their primary care provider the following day or soon after.

At Telluride Regional Medical Center, Paula’s work embodies what dedicated care management looks like. “My goal is for patients to fully understand pre-diabetes and diabetes,” Paula explains. “I want them to become ‘best friends’ with it. When people understand the condition, they are empowered to manage it effectively.”

Now, as a certified CDCES and bilingual educator, Paula is expanding access to high-quality, culturally responsive diabetes care—especially for Spanish-speaking and underserved patients in the region. Her work is instrumental in helping patients improve their quality of life, prevent disease progression, and reduce the risk of complications.

“We’re incredibly proud of Paula’s achievement,” said Dr. Sharon Grundy, Medical Director at Telluride Regional Medical Center. “Her experience, compassion, and dedication are a tremendous asset to our team and to the community we serve.”
