NOTICE OF OPPORTUNITY TO CLAIM MEDICAL RECORDS

NOVEMBER 1, 2021

PURSUANT TO CHAPTER 22 TEXAS ADMINISTRATIVE CODE SECTION 165, ON OR ABOUT SEPTEMBER 20, 2021, NOTICE OF TERMINATION WAS GIVEN FOR THE MEDICAL PRACTICE KNOWN AS THE WOODLANDS LUNG CENTER.

THIS ADDITIONAL NOTICE IS BEING PROVIDED TO ALL PATIENTS WHO WISH TO OBTAIN A COPY OF THEIR MEDICAL RECORDS OR HAVE SUCH RECORDS TRANSFERRED. ALL PATIENTS HAVE 30 DAYS FROM THE DATE OF THIS NOTICE TO REQUEST SUCH RECORDS AND ARE HEREBY NOTICED THAT AFTER THAT DATE THE RECORDS WILL NO LONGER BE AVAILABLE.

Dear Patients,

MEMORANDUM OF RECORD
FOR PATIENTS OF DR. CARLOS ARAUJO

To begin, we would like to thank you for the trust you have given our practice over the years for your pulmonary care. Taking care of you and your family was an absolute honor for the Araujo family and our staff at The Woodlands Lung Center and Houston Intensive Care and Pulmonary.
After the unexpected passing of Dr. Carlos Araujo on November 30, 2020, the Court approved a new Custodian, Dr. Juan Carlos Zubieta, to oversee the transition of patient care at the practice. Over the past several months, Dr. Zubieta has worked to review current and previous medical management and/or cases; make decisions as to the medical management of patient care; and administer the proper transition and storage of your medical records.
This letter serves to notify you of the permanent closure of the Woodlands Lung Center, and to ensure you are aware of your options related to the transition of care and how to access your medical records going forward.
Practice Closure Date: October 14, 2021
Please note that all phone and fax numbers will also be terminated.

Dr. Zubieta is accepting patients at his existing practice, and you can contact him at the number listed above regarding any continuing care needs. You can also transition your care to a new medical provider. Custodianship of all medical records that were previously stored in Tomball or The Woodlands, have been transferred to Dr. Zubieta. If you would like to request a copy of your medical records, or have your medical records transferred to another provider, you can call our office until October 14, 2021, and after then please contact the new Custodian of Medical Records at the number listed above. Medical records are stored in accordance with Texas Administrative Code regulations for a minimum of seven years from the anniversary date of the date of last treatment by the physician.

TRACHEAL TEAR

Following routine endotracheal intubation, the incidence of TBI is low, approximately 0.005 percent for single-lumen intubation and 0.05 percent for double-lumen intubation [7]. However, the incidence can be as high as 15 percent following emergency intubation [8]. Finally, the incidence of TBI following percutaneous tracheostomy placement is approximately 0.2 percent
