- Connect Care Management is looking to hire a licensed clinical social worker.
- Candidate will perform patient outreach pertaining to Behavioral Health Integration.
- Candidate will assist in developing and implementing the necessary protocols for BHI.
- Candidate must have past experience in counseling or mental health.
- Majority of the job will be performed without face to face interaction.
- Candidate needs to be able to type and be able to work with multiple different EHRs.
- This position will eventually grow into a management position.
