Are you or your loved one having difficulty with your normal activities at home? Perhaps you or your loved one are not walking as well, feeling weaker than usual with poor endurance? Perhaps there have been some near falls or an actual fall?

With a doctor’s referral, you may qualify for home health therapy evaluations. Once the evaluation is completed, the therapist will set up a plan of care for your recovery and safety in your home.
