We are committed to providing the highest quality, patient-centered hospice services to our patients, and to assist them in achieving the highest quality of life possible during their final season of life. Our qualified, trustworthy and compassionate personnel are committed to going above and beyond in their efforts to serve those in our care. The Agape Care Team
also includes physicians, RN Case Managers, Social Workers, Chaplains, Certified Nursing Assistants and volunteers. The team collaborates to create a comprehensive, individualized care plan for each patient and their loved ones. This plan of care addresses not only the physical needs of the patient, but their emotional, social and spiritual needs as well. In addition, this team approach not only focuses on the emotional needs of the patient, but their loved ones as well. Each team member has a unique roll to ensure the utmost compassionate care possible. The RN Case manager works closely with the physician and certified nursing assistant to ensure that physical needs are met. If the need arises, to help keep our patient comfortable the care team may include other specialist, including speech, physical and occupational therapists,and dietitians. The team also includes on-call nurses to help us provide quality care 24 hours a day seven days a week. Our patient psycho-social, emotional and spiritual needs are addressed by our the social worker, chaplain and volunteers.
