Reminder:

If you are using your soonercare for a discounted fee, you have to provide PROOF of soonercare. That means a CURRENT card. The old ones are not being accepted. If you don't have a card, you can send in a screenshot off of the soonercare website.

Someone told me they found a place that doesn't charge $104.30 for the state fee.

The state fee is the same no matter who helps you with your card. Unless they are paying the state fee for you.

If you answered YES to having Medicare, SoonerCare, or being a 100% disabled veteran, you qualify for the $22.50 state fee.

If you selected YES but then provided Blue Cross or another private insurance, your form contains incorrect information. Please double-check your answers so we can avoid delays or confusion in processing.
