We have been honored to walk alongside patients and families during some of life’s most sacred moments. We are deeply thankful for the trust placed in us, the compassion shown by our caregivers, and the strength and love demonstrated by every family we serve.

To our dedicated team, community partners, and the families who allowed us into their lives—thank you. Your courage, kindness, and support inspire everything we do.

As we reflect on this year, we carry forward the stories, lessons, and love that remind us why this work matters.

We had such a wonderful time participating in the CMSA-OK Annual Conference in Oklahoma City on November 12th & 13th! This year’s theme — Mad Hatter’s Tea Party — made for a fun and creative way to celebrate the incredible work of Case Managers across Oklahoma.

Our TruHeart Home Health & Hospice team loved connecting with so many passionate professionals who share our mission of providing compassionate, patient-centered care.

A big thank you to everyone who stopped by our booth — we appreciated every conversation, every smile, and every opportunity to share what makes TruHeart special.
