If you need to request a copy of your child’s medical records, all requests must be turned in by Wednesday, May 7th. Requests can be emailed to mpbell@nmhs.net or faxed to 662-840-6030. 😊

If you have received handwritten prescriptions for your child's ADD/ADHD medication, take all of them to your pharmacy and make sure that they will be accepted. If there are any questions, please call our office as soon as possible.

Regarding medical records-if you have gotten the form and sent it back, we are working quickly to get those back to you. If you want a paper copy of the records, there is a $10.00 fee. Due to the high volume of requests, we will not be mailing or faxing the records.
