"On January 30, 2026, government funding will expire—and with it, therapists and other clinicians will lose two critical Medicare policies unless Congress acts:
1. COVID-19 era telehealth flexibilities that allow therapists to provide rehab therapy via telehealth.
2. The 1.0 Work Geographic Practice Cost Index (GPCI) floor, which protects clinician reimbursement nationwide.
The House of Representatives has already passed an appropriations bill that would extend both of these policies. However, the bill must still be passed by the Senate to prevent another lapse.
The current flexibilities given to therapists to provide telehealth is on the brink of expiration. Without it, many Medicare beneficiaries would lose access to rehab therapy services. Additionally, the expiration of the 1.0 Work GPCI floor would result in further reductions to clinician reimbursement, creating additional financial pressure on providers.
Importantly, both of these policies previously lapsed during the October 2025 government shutdown, leading to gaps in coverage, confusion among beneficiaries, and added administrative and financial strain on providers. Congress should not allow this disruption to occur again.
