IMPORTANT REMINDER: Medical records will be available until August 25th, 2025. You must request a copy of your medical records beforehand by sending a written request to our office at:

Please include the patient’s full name, date of birth, contact information, and signature. We will notify you as soon as your medical records are ready to be picked up. We can not accept requests on August 25th, so please make sure you get your requests in now so that your medical records will be available by that date. After August 25th, records will be maintained off site and will be properly destroyed after such time the Pennsylvania law regarding medical records retention requires.
