What is Incident to billing?
"Incident to" allows billing of services and supplies performed by auxiliary personnel and are reimbursed 100% of the insurance fee schedule.
Medicare defines a provider to include nurse practitioners, clinical nurse specialists, certified nurse midwives, physician assistants, clinical psychologists, clinical social workers and physical and occupational therapists.

The supervising provider must first see the patient and develop a plan of care and initiate the course of treatment. The “incident to” service provided by the auxiliary personnel is then an incidental part of the patient's treatment. The patient can see the auxiliary personnel for continued treatment of the initial problem that was presented to the provider. If a new problem is identified at a visit by the auxiliary personnel, the patient must be referred back to the provider for evaluation and development of a new plan of care. The provider must demonstrate an active participation in the ongoing care of the patient, such as providing services on a regular basis that reflects participation on an ongoing basis. Reimbursement is based on 100% of the physician (except CSW supervisors)fee schedule amount.
Certain requirements must be met to bill “incident to."

Beginning January 1, 2022, if you’re uninsured or you pay for health care bills yourself (don’t have your claims submitted to your health plan), health care providers and facilities must provide you with an estimate of expected charges before you get an item or service. This is called a “good faith estimate. Providers and facilities must:

Provide the good faith estimate before an item or service is scheduled, within certain timeframes.
Offer an itemized list of each item or service, grouped by the provider or facility offering care. Each item or service has to have specific details, like the health care code assigned to it and the expected charge.
Explain the good faith estimate to you over the phone or in-person if you request it, and then follow up with a written (paper or electronic) estimate.
Provide the good faith estimate in a way that’s accessible to you.

Starting in 2022, new pricing information will be shown on any physical or electronic plan or insurance identification card (ID) provided to patients.

Including:
Applicable deductibles
Applicable out-of-pocket maximum limits
A telephone number and website where you can get help access through a Quick Response code (commonly referred to as a QR code) on a physical ID card, or through a hyperlink on a digital ID card.
