Ivy Creek Hospice was proud to partner with Southside Middle School in Tallassee to provide sports physicals for student athletes preparing for the upcoming sports season.

Our team always enjoys opportunities to support the health and well-being of the young athletes in our community. Ensuring students are ready and safe to participate in sports is just one of the many ways we love giving back.

A special thank you to the students, parents, coaches, and school staff, as well as Dr. Melissa Thompson and Leslie Pendergrass, CRNP for helping make this event possible and allowing us to be part of such an important day.

We wish all of the athletes a safe, successful, and exciting season!

Position: RN Case Manager – Hospice
Locations: Ivy Creek Hospice – Dadeville & Elmore, AL 

What You’ll Do:
✨ Provide and coordinate patient-focused hospice care
✨ Ensure safe, high-quality nursing services that reflect our hospice philosophy
✨ Support patients and families during meaningful end-of-life care

Qualifications:
✅ Graduate of an accredited nursing program
✅ Licensed RN in the State of Alabama
✅ At least 1 year of clinical nursing experience (or completion of agency preceptor program)
✅ Strong communication, organization & teamwork skills
✅ Ability to travel between locations as needed
