OPWDD has approved eight provider agencies to provide Home‑Enabling Supports (HES) to people with IDD who live in the community and are enrolled in OPWDD and the HCBS Waiver.

These supports include assistive technology, remote supports, and Health Assessment & Coordination Services — designed to boost safety, independence, and meaningful community involvement.

With support from his LIFEPlan CCO care manager, Sabrinna, Stephen returned to New York City to reconnect with his roots, culture, and memories.

From coordinating logistics to securing funding and supports, this journey shows what person-centered care management can make possible.

Stephen continues to live independently through Family Care supports, attend day habilitation, and earn income from knitting- a skill he uses for therapy and productivity.

His story is a reminder that with the right support, people with IDD can pursue goals at any stage of life.
