A Registered Nurse begins each assessment with a personal visit to the house and/or health care facility. Once the Personalized Assessment and medication profile is completed, our Nurse will discuss the assessment with you and exchange ideas to determine what Plan of Care is needed. This will ensure the level of medical and non-medical care meets your expectations. Depending upon your needs, we usually can begin service within 24 to 48 hours after the assessment is completed. In some cases, we are able to begin a personalized Plan Of Care the same day. We offer convenient flexible scheduling including:
- Hourly Care from 4-24 Hours per Day
- Personal Care Visit from 1-1 ½ hours (sometimes called a bath visit)
- Respite Weekends
Our experienced caregivers will customize a plan that is suitable for you and your family, whether it is 1 day per week or 7 days per week. There is no contractual obligation for the amount of time services are rendered. Rest assured Relevar’s warm and compassionate staff will fulfill your needs for as long or short a time as you need.
