Last year, we received a generous grant from Tri-County ADAMHS Board - Mercer, Paulding and Van Wert Counties to help with our 2026 Rays of Hope Grief Camp, this June 8th, 9th, & 10th.☀️
Rays of Hope Camp is a community outreach program through CHP Home Care & Hospice for kids ages 6-17 who experienced the loss of someone close to them. Camp is lead by a team of social workers, educators, and nursing staff and includes includes learning and activities like swimming, crafts, group time, games, rock climbing wall & more. Camp is FREE thanks to support from Tri-County ADAMHS, Noah's Ark Fund, and various community groups.

🌟 CHP Home Care & Hospice is honored to receive a $5,000 mini grant from Tri County ADAMHS in support of our annual Rays of Hope Grief Camp in Van Wert.

Grief Camp each summer provides a safe space for local youth to navigate their emotions and find healthy ways to cope with sadness and grief after experiencing loss. Thanks to this generous support, we can continue offering hope and healing to the young people in our community and make a lasting impact in their lives. We are truly grateful to Tri County ADAMHS and their support.

🩺 Blessing of the Hands symbolizes the connection between the hands and the heart in the delivery of health care. CHP Home Care & Hospice is offering this brief moment of blessing at local health care providers during National Nurses Week, May 6-12. A CHP Hospice chaplain will come to local hospitals and facilities to "bless" their staff and honor their vital role in patient care. Contact Cindy or Alycen at 419-238-9223 to schedule. 🙏❤

CHP Home Care & Hospice is proud to highlight Alycen Urban, an Event Coordinator & Marketer as the 2026 President of the Board for the Paulding Area Chamber of Commerce -Ohio. Alycen has been at CHP’s Corporate office for 5 years and also assists with grant writing to support nonprofit programs.

She holds a Bachelor’s Degree in Mass Communications with a minor in Sociology and felt called to hospice care after witnessing the compassion her grandmother received through CHP. Alycen is passionate about raising funds for local patient care and representing rural communities like her hometown of Van Wert, Ohio.

“In 2023, I was approached to serve on the Board of Directors for the Paulding Chamber and was honored to be part of a wonderful group of community members who are passionate about supporting local businesses and organizations.”

Outside of work, Alycen is engaged to her fiancé, Jordan, and is a proud mom of four. She enjoys family time, church, strength training, cooking, and watching her kids play sports.
