Legion Commander, Kyle McKelvey presented a check to CHP President/CEO, Brent Tow. Our patient care fund helps pay for expenses when there are coverage deficiencies. On GivingTuesday, The Van Wert County Foundation will again match contributions to the Fawn Burley CHP Hospice Patient Care Fund - up to $10,000. Your donation - no matter the amount - is doubled!! In order for your donation to be matched, it must be given through the VWCF. Here's how:

Online
• vanwertcountyfoundation.org/give. Click “Make a Giving Tuesday Donation” and scroll to the CHP logo and Fawn Burley CHP Hospice Patient Care Fund

In-Person
• On Dec. 2, you can pay by cash or check when you donate at the Van Wert County Foundation office, 138 E. Main St., Van Wert.

Mail
• You may mail a donation prior to Giving Tuesday and VWCF will hold the check and complete the donation on the actual day of giving. Note Giving Tuesday in your correspondence.

*If paying by check, it must be made payable to the Van Wert County Foundation with the preferred fund (Fawn Burley CHP Hospice Patient Care Fund) listed in the memo.

The Van Wert County Foundation will match donations up to $10,000 that day to benefit the Fawn Burley CHP Hospice Patient Care Fund. Giving Tuesday donations can be made online at vanwertcountyfoundation.org, or by check or cash delivered to The Van Wert County Foundation, 138 E. Community Health Professionals (CHP) Home Care & Hospice is a non-profit home health & hospice organization serving 15 counties in Northwest and West Central Ohio.

What makes CHP unique is that we provide a complete range of services - including skilled home health nursing, in-home therapy, hospice, inpatient hospice, non-clinical home care, and adult day services.

Our nurses and staff who are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
