At Fountain Park Home Health, we help make the transition from rehab to home as smooth and successful as possible. Every resident discharged after a rehab stay at a Vancrest facility receives a home health referral, and we’re honored to be one of the options they can choose.

What makes our care different

🎉 It’s a week of celebration for Pam Motycka! 🎉

For her last week at Vancrest, we’re honoring Pam’s 21 incredible years of dedication, hard work, and countless smiles. Today was Sports Day, and staff wore their favorite team gear and played co****le with Pam.

To make the week even brighter, Pam received a beautiful flower bouquet from her sisters to enjoy in the office! 🌸💛

We’re so grateful for everything Pam has done and wish her a retirement full of joy, laughter, and exciting new adventures!
