Don't Leave It To "Opinion" - Get It Checked!
This is a common misconception by doctors, PT's, and even chiropractors, who do not extensively deal with severe Spinal Disc problems such as DISC HERNIATIONS , SPINAL STENOSIS or DEGENERATIVE DISCS on a day to day basis. Much of this goes back to what doctors learned in school. If you have leg pain (SCIATICA), you have a disc problem ---- if you do not have leg lain, your disc is not the problem. The same thing can be said of the relationship of the Spinal Disc to low back pain. I actually see a fair number of people who have severe buttock / leg pain, yet have no low back pain whatsoever.

Is it from the Disc, or is it something else?

Just like individual fingerprints, the reality is that no two bodies are exactly the same. Professors present lectures, and text books present anatomical pictures of the way that various structures in the body should look. We know how different people look from one another on the outside. The truth is that because everyone is so different on the inside, nerves are not always found exactly where one would expect them to be. In fact, when it comes to studying human anatomy, anomalies and anatomical variations are the norm. Discs can herniate in any number of common (or very uncommon) patterns. When you throw in the phenomenon of ASYMPTOMATIC HERNIATIONS, the conundrum gets even bigger. You may be starting to understand why back surgeries can be such a crap shoot.

FAILED BACK SURGERY SYNDROME PAIN:
How Common? Chiropractic Helped Reduce Pain and Opioid Use
Researchers stated that degenerative disc disease and low back pain turns into chronic low back pain for many after back surgery discectomy. They also reported that nearly 15% of back surgery patients had failed back surgical syndrome (FBSS) with back pain extending beyond 6 months post-surgery. (1) Chiropractic care is well-known for its conservative treatment of back pain and neck pain.
Now, the chiropractic Cox® Technic System of Spinal Pain Management, Used by Vancouver Disc Center of America, is showing itself as beneficial for relieving post-surgical FBSS spine pain.
A retrospective study of 69 such patients treated by 15 chiropractors experienced in applying Cox® Technic protocols documented greater than 50% relief of pain for 81% of patients in a mean of 11 visits over a 49-day period of active care.
This relief at the end of care continued at 2 years follow-up: 78.6% continued with relief. The percentage of relief remained high: 71.6% at the end of care and 70% at 24 months. Spinal hydration refers to the fluid content within the intervertebral discs, particularly the nucleus pulposus, which is composed largely of water bound to proteoglycans. In a healthy disc, hydration levels can approach 70–90% in youth, allowing the disc to function as a viscoelastic shock absorber. This water content enables the spine to distribute compressive loads evenly, maintain disc height, and preserve foraminal spacing for exiting nerve roots. Disc hydration fluctuates diurnally—typically higher in the morning after recumbency and reduced by the end of the day due to axial loading.

Loss of spinal hydration is a central component of degenerative disc pathology. Reduced proteoglycan concentration impairs the disc’s ability to attract and retain water, leading to diminished intradiscal pressure, annular stress, and progressive disc height loss. Biomechanically, this alters segmental load transfer, increases facet joint stress, and may contribute to bulging, herniation, or spinal stenosis. Clinically, patients may present with axial pain, radiculopathy, stiffness, or reduced mobility. MRI T2-weighted imaging commonly reflects hydration status, with darker discs indicating desiccation.

Strategies that support spinal hydration focus on mechanical unloading, controlled movement, and systemic hydration. Interventions such as spinal decompression therapy aim to reduce intradiscal pressure, potentially facilitating nutrient diffusion and fluid reabsorption through the cartilaginous endplates. Regular movement enhances diffusion-based nutrient exchange, as discs are largely avascular. Adequate water intake, anti-inflammatory nutrition, and load management further support disc matrix integrity. While structural degeneration cannot always be reversed, optimizing disc hydration may improve biomechanical function and symptom expression.
