Does anyone have a list of QUALIFIED NURSE ASSESSORS in Clark County?
I'm working on a DD placement currently & potentially will be working on a couple from Southern Oregon moving to the Vancouver Area to be closer to their son.

I’m looking for a 58 year old lady that will be on Medicaid within a year that’s DD, Schizophrenia Bipolar who is living with her mother. Mom is no longer able to provide her care. She’s in her 80’s and looking down the road to moving into an AL soon.
Daughter is on depakote and zaprazadone. Likes to be left alone. Is fairly independent still. Just prefers isolation.
Mom is looking for a home where other DD residents are at that MAYBE the daughter might relate with. Prefers to have her own room too.
Area of town isn’t that important but would be nice if it’s in the HD or Mall area.

My friend’s daughter is in the hospital and on a TRAEC and a VENT.
Options are seemingly limited to four locations.
Family isn’t impressed with any of the options.
I know there’s some VENT HOMES in Vancouver. Does any of them have any openings?
Daughter will be Medicaid. Need repositioning q4h.

What really is a Placement Service

We have received several comments/concerns regarding MOVE IN'S during the COVID 19 Outbreak.
I have "good news" to report.
I did a little research on our Adult Family Home Association of Washington's website & found the following:
Admissions
• No restrictions on admitting new residents (be sure to update your openings on the AFH Council’s ‘Find a Home’ locator page)
• Virtual tours are recommended since in-person visits are prohibited
• Hospitals are unable to provide Covid-19 testing prior to admission in the home due to scarcity of tests
• Upon move-in family should be aware they will not be able to visit to help with transition during this time other than via video visitations
• Recommend limiting number of personal belongings a resident brings with them until restrictions are lifted
• Consider enhanced social distancing in addition to daily screening for two weeks following admission to monitor for Covid-19 symptoms
Assessments
• Management Bulletin MB 20-017 for information about assessments
• Assessment will be completed by phone or using other technology whenever possible
• If there is a concern for client health or safety or a client requests a visit, Case Managers will use their professional judgement to determine how to respond
• Private pay assessments: there is no requirement for these to be in-person (WAC 388-76-10335 through 10350)
Alternative Options is prepared to continue placements of residents using all the recommendations & precautions that the CDC & DSHS recommend.
We want the AFH, Providers, Residents & Families to all BE SAFE and yet still provide a much needed service for those that are searching for ALTERNATIVE OPTIONS for their elderly loved ones.

I’ve got a placement looking for AFH in the cascade park area. 85 yo lady with ALS. 5”6’ 135lbs. Loves kids, pets, activities. Needs assistance with transfers, bathing, toileting, med management and dressing. Uses a wheelchair and in the process of getting an electric. Very lovely lady. Message for details.
