If you have been dealing with chronic neck, back, or joint pain, your provider may recommend a procedure called Radiofrequency Ablation (RFA).

RFA is a minimally invasive treatment that uses controlled heat to interrupt pain signals from specific nerves. These nerves send pain messages from irritated joints in the spine or other areas of the body to the brain.

By gently applying heat to the nerve, we can help reduce or stop those pain signals, which may provide longer-lasting relief than injections alone.

RFA is commonly used to treat pain from:

• Arthritis in the spine (facet joint pain)
• Chronic neck or low back pain
• Some types of joint pain in the shoulders, knees, or hips

The procedure is done with X ray guidance and usually takes less than 20 minutes. Patients are able to go home the same day and return to normal activities shortly afterward.

For many patients, relief from RFA can last months to even over a year, depending on the individual.

We appreciate those who took the time to come out and learn more about chronic pain, the different types of pain patients experience, and some of the treatment options available to help improve quality of life.

Therapy Thursday: Spinal Cord Stimulator Trial

For some patients who continue to experience pain after back surgery, a treatment called a Spinal Cord Stimulator (SCS) may help reduce nerve pain.

A spinal cord stimulator works by sending small electrical signals to the spinal cord that help interrupt pain signals before they reach the brain. Like a pacemaker for pain.

One of the unique aspects of this treatment is that it begins with a trial.

During the trial (that happens in the office procedure room under X ray):
• Temporary leads are placed near the spinal cord
• The patient goes home and tests the device for several days
• If the pain improves significantly, a permanent stimulator may be considered

Many patients describe spinal cord stimulation as helping them move more comfortably and rely less on pain medications.

This therapy can sometimes help patients with:

• Pain after back surgery
• Chronic nerve pain in the back or legs
• Painful diabetic neuropathy

Pain After Back Surgery

Many people undergo back surgery hoping it will completely relieve their pain. While surgery can help many conditions, some patients continue to experience ongoing nerve pain afterward.

This is sometimes called Post-Laminectomy Syndrome or Failed Back Surgery Syndrome.

Common symptoms may include:

• Persistent low back pain
• Burning or shooting pain down the leg
• Numbness or tingling in the leg or foot
• Pain that worsens with activity or standing

There are many reasons this can happen. Scar tissue may form around the nerves, the original problem may continue to irritate the nerve, or new issues can develop over time.

The important thing to know is that you still have treatment options, even if surgery did not completely solve the problem.

At Interventional Spine & Pain Institute, our physicians evaluate patients carefully to determine the cause of ongoing pain and recommend treatments that may help improve comfort and function.

What Are Endplate (Modic) Changes on an MRI?

If you’ve ever had an MRI of your spine, your report may mention something called “endplate changes” or “Modic changes.”

The vertebral endplates are the areas where the disc meets the bone of the spine. Over time, inflammation and stress in these areas can cause visible changes on MRI. These changes are often associated with a specific type of back pain called vertebrogenic pain.

People with vertebrogenic pain often describe:

• Deep aching pain in the lower back
• Pain that worsens with sitting for long periods
• Pain when bending forward or lifting
• Pain that improves when standing or walking

For some patients, a minimally invasive treatment called basivertebral nerve ablation (the Intracept procedure) may help.

This procedure works by targeting the basivertebral nerve, which carries pain signals from the inflamed endplates inside the vertebra. By interrupting this nerve signal, many patients experience meaningful and lasting relief.

At Interventional Spine & Pain Institute, our physicians perform this procedure for carefully selected patients with vertebrogenic pain confirmed on MRI.

If you’ve been told you have Modic changes or endplate inflammation, it may be worth discussing whether vertebrogenic pain could be contributing to your symptoms.
