Virtual Exercise Rehabilitation Assistant (VERA) was recently cited and utilized in a
study conducted by Duke Clinical Research Institute. This study compared the costs
associated with physical therapy following a Total Knee Arthroplasty (TKA) provided by
a physical therapist or through a virtual assistant. The study reported a large cost savings
per patient and stated “the use of VERA was as effective as traditional PT”. I have many
concerns about this virtual exercise program during a critical period following a TKA. It
is imperative that inflammation and pain are reduced, joint flexibility and motion are
restored, and strength improves to allow for a return of functional mobility. This process
needs to occur in a specific time frame so that adhesions do not form, which physically
restrict range of motion. This can require additional intervention (and costs, as well as
recovery time) from the surgeon using manipulation under anesthesia to alter the
adhesions. Some clients need more encouragement and hands-on care, while others need
cues to “slow down”. Modalities, kinesiotape, lymphatic drainage, manual techniques
and scar mobilization require in-person care and instructions. Physical Therapists are
essential for the hands-on rehab process and the utilization of their skills to adapt the
program as necessary. One should proceed with caution when relying solely on a virtual
assistant for recovery from a significant surgery.
