One of the greatest gifts you can give your family is clarity.
An advance directive lets you put your healthcare wishes in writing so that if you're ever unable to speak for yourself, your voice is still heard.

It covers important decisions like:
-Do you want life-sustaining treatment?
-Who will make decisions on your behalf?
-What does a "good death" mean to you?

These conversations aren't easy - but they are powerful. Our team can help guide you through the process.

Behind every family we serve is a social worker making sure nothing falls through the cracks.

Our social workers connect families with the benefits and resources they need so they can focus on what matters most: time together.

They help families:
✅ Understand insurance & Medicare/Medicaid benefits
✅ Connect with community resources
✅ Plan for what lies ahead with confidence

At Community Hospice, no family walks alone.

Off The Bookshelf — Books We Love for the Journey

"I Miss You: A First Look at Death" by Pat Thomas is a gentle, honest picture book that helps young children understand death and grief in age-appropriate language.

Losing a grandparent, a parent, or someone dear is confusing for little ones. This book opens the door for important conversations - with warmth, not fear.

If you're helping a child in your family process loss, this is a beautiful place to start. Ask about the grief resources available through Community Hospice.

Not sure if it's "time" for hospice? A free consultation is a great place to start - with no obligation and no pressure.

Here are questions worth asking:
-Does my loved one qualify for hospice?
-What services are included at no cost to our family?
-Can our current doctor still be involved?
-What happens if their condition improves?
-How quickly can care begin?

We welcome every question. Our team is here to inform and support - never to rush you.
