Southern Choices, LLC Community Supports Services offers specialized services in several regions and is comprised of the following programs:
1) Community Residential Services
-Developmentally Disabled
-Dual Diagnosed
2) Community Living Services
Personal Care Home
-Provided for developmentally disabled.
-Trained staff provides watchful care and training in daily living skills, money management and responsible community behavior. Semi-Independent
Living Arrangements
-Designed for adults who have the necessary basic skills to live without 24-hour supervision and live with families.
-Outcome is for these individuals to live as independently as possible. Supportive Living
-Provides assistance to individuals who need help with placement and/or the supports necessary to maintain a stable living situation.
-Assistance with securing an appropriate placement based on skills necessary to maintain placement.
-Services include: help with budgeting, housekeeping skills, shopping, health maintenance & nutritional education, transportation to medical appointments, assistance with obtaining and managing prescribed medications, and accessing community resources.
