Case of the Week:
We go to Cortina and Lindsay Vonn this week. The erector set that was placed on the patient's leg stabilized a complex fracture with many pieces of bone. In oral and maxillofacial surgery we use the same technique when fractures are complicated and cutting risks compromise of blood supply leading to dead bone or bone that is simply missing from a bullet or blast injury. Pins are placed through the skin above and below the fracture then the pins are stabilized with external metal framework to immobilize the fracture. The fracture can then be definitively treated with plates and screws at a later date. Dr. Hacket in Vail, CO helped me with a complex clavicle fracture I sustained from a skiing accident 2 years ago and has been by Lindsay's side since tearing her ACL 2 weeks ago. Today her leg was opend and definitively treated with long titanium plates and screws. Praying for no infections after multiple procedures. World class operators doing miracle surgery saving a warrior's leg on two continents.

Case of The Week
A 76 year old female presents with a white patch on the left lateral tongue. She does not smoke or drink. She has no symptoms and states her Dentist found it on routine exam. I biopsied the lesion and a diagnosis of Moderate Dysplasia was made by the Oral Pathologists at VCU. Dysplasia is a pre malignant lesion that comes in 3 flavors before becoming cancer....mild, moderate and severe. I recommended laser fulgaration of the left lateral tongue to vaporize the tissue then allow it to heal for a month and grow new healthy tissue. The patient is cured now but under tight surveillance by me, her dentist and herself for the next 5 years.

Case of the week.
72 year old female who lost her right lateral incisor and failed an implant in another doctor's office. The width of the maxilla was only 1mm requiring bone reconstruction. An implant was placed and grafted with a new graft material composed of human donor bone in a slurry of lineage-committed bone cells creating scaffolding and stimulation to form new bone. The graft was then covered in a bioresorbable membrane barrier. This is next level grafting!

Case of the week...
77 year old female underwent uneventful excision of a left soft palate lesion that was sent for pathology. The soft tissue was cauterized with silver nitrate to stop any bleeding. The next day her husband sent these photos of his wife with normal healing in the soft palate and bilateral upper and lower tire balloon lips. What is the diagnosis and treatment?

Case of the week:
It's increasingly important to scrutinize your doctors training and complication rate for specific procedures. There is more and more mediocrity in the community with bigger groups and more locations and large corporations owning practices. I'm seeing, fixing and serving as an expert witness in more complications than ever! Words to the wise!

Summer is the best time to remove wisdom teeth because high school age kids have time to heal. Removal of wisdom teeth BEFORE roots are fully grown results in the lowest chance of complications and nerve injuries. Always seek a 2nd opinions by a Board Certified Oral and Maxillofacial Surgeon whenever surgery is being considered in the mouth.
