Position Overview
We are seeking a dedicated and compassionate Case Manager to join our team in Wakefield, MA. The Case Manager plays a vital role in coordinating care, supporting clients through treatment, and ensuring continuity of services from admission through discharge and aftercare. 🤝
Key Responsibilities
• Complete biopsychosocial assessments and ongoing case management documentation 📝
• Develop and coordinate discharge plans 📋
• Link clients to community resources, outpatient providers, sober living, and recovery supports 🔗
• Facilitate Groups
• Facilitate referrals, insurance authorizations, and aftercare planning 📞
• Advocate for clients and families throughout the treatment process 💬
Qualifications
• Minimum 1–2 years experience in substance use disorder or behavioral health treatment 🌱
• Knowledge of levels of care (PHP, IOP, OP, residential) and ASAM criteria 📊
• Strong organizational, communication, and documentation skills ✍️
• Ability to work collaboratively in a team-based, fast-paced clinical environment 🤝
