A full-time (1.0 fte) opportunity is available for a Case Manager in the Outpatient Department. This position will be responsible for assessing consumer needs, developing, implementing and reviewing individual plans of services, and working with other providers and community resources in meeting/achieving consumer service needs.
The successful candidate must be 18 years of age, possess a bachelor’s degree in social work and a license as such in the state of Michigan, or ability to obtain a license as a Licensed Bachelor’s Social Worker or Limited Licensed Bachelor’s Social Worker. A graduate from an accredited school of nursing who is licensed and a Registered Nurse in the State of Michigan is also qualified to apply for this position. Two years of relevant experience in a mental health agency preferred. Equivalent experience will be considered in lieu of bachelor’s degree to fill as a Supports Coordinator Assistant. This position is responsible for managing an assigned caseload of consumers aged 0-20 in the children’s department. This position will be responsible for assessing consumer needs, developing, implementing and reviewing individual plans of services, and working with other providers and community resources in meeting/achieving consumer service needs.

The successful candidate must be 18 years of age, possess a Bachelor’s Degree in Social Work and a license as such in the state of Michigan, or ability to obtain a license as a Licensed Bachelor’s Social Worker or Limited Licensed Bachelor’s Social Worker. A graduate from an accredited school of nursing who is licensed and a Registered Nurse in the State of Michigan is also qualified to apply for this position. Two years of relevant experience in a mental health agency preferred and an interest in serving the infant/early childhood population. Must possess a valid Michigan or Wisconsin Driver’s license.
