Forte Residential, Inc - Forte Home Health Care, Inc. Official page of Forte Residential & Forte HHC. Forte Residential, Inc is a certified Medicaid Waiver Provider in the state of Indiana.

Forte assists and supports persons who are developmentally disabled or otherwise physically challenged through family driven initiatives to manage their own affairs in their own homes. We provide services to recipients of the Community Integration and Habilitation (CIH) Waiver, Family Support Waiver (FSW), and Aged & Disabled (A&D) Waiver. Forte Residential, Inc provides Residential Habilitation Services (RH1O, RH2O), Community Habilitation (CHIO), Respite Services, Attendant Care and Homemaker Services as allowed by each respective waiver. Forte Residential also provides Caregiver Support Services. Forte Residential is currently approved to provide waiver services in 31 northern Indiana counties. Forte Home Health Care provides Medicaid PA (home health aide and skilled nursing) services for individuals requiring assistance in their homes. Forte Home Health Care provides services in 21 counties in Indiana. We work with each individual and their unique situation to pursue a barrier-free, fulfilling, safe, productive, and enjoyable life.

A Message from The Division of Disability, Aging, and Rehabilitative Services: DDARS Advisory Council Family Member Opening and Application

The Division of Disability, Aging, and Rehabilitative Services is excited to announce the DDARS Advisory Council is recruiting a new member to serve in the immediate or extended family member of an individual with an intellectual or other developmental disability role. Membership representation of this Advisory Council is established in Indiana Code.

General Overview of the Council’s Purpose
The DDARS Advisory Council was established under Indiana Code Section 12-9-4 to advise and assist DDARS in its effort to develop and sustain a system of supports and services for individuals with intellectual and developmental disabilities. The Council provides expertise, lived experiences, and advice on specific areas such as: technology; health; policy; law; marketing; public relations; provider services; and advocacy. The Council is individual and family-driven and helps carry out DDRS’ mission to support a wide variety of Hoosiers with disabilities to create a vision for their future that uses paid and natural supports to build on and enhance their personal strengths and assets.

General Membership Overview
Membership is established through Indiana Code Section 12-9-4-3. This particular position is appointed by the DDARS Director and will serve a two-year term, beginning in February, 2026.

General Meeting Information
The DDARS Council meetings are held six months throughout the year on the third Wednesday of the month from 9:30 a.m. - 12:30 p.m. The majority of the meetings are held in-person at the Indiana Government Center, Indianapolis, Indiana.

Support for Participation
The Family Member appointed to the Council in this role is provided with reimbursement for parking expenses, as needed. Additionally, DDARS will provide reasonable accommodations in connection with the member’s duties as provided in state policies and procedures.

Selection Process
Upon receiving your application, DDARS will review your application, and individuals meeting the role requirements will be screened for potential membership. It is possible you will be contacted for a virtual interview with DDARS staff members. Applicants who are not appointed are invited to stay actively engaged in DDARS activities.

What is this about?

The Indiana Bureau of Disabilities Services is planning to create new home and community-based service waivers to improve services. This is known as the “waiver reset.”

In 2027, BDS plans to move people accessing services on the current four waivers into the new waivers. BDS is still working on figuring out what the new waivers will be like.

To help, BDS has been working with the Human Services Research Institute to host conversations about what changes are needed. Based on the feedback received, HSRI will develop recommendations for the waiver reset.

During the December 5 call, HSRI will provide an overview of the waiver reset, answer questions, and listen to feedback from the community about suggested changes to the waivers.
