Warsaw Meadows Skilled nursing facility offering advanced dementia care, short term rehab and general nursing care services. From the North, take SR15 south through town.

Warsaw Meadows is located south of US 30 just off of SR 15. Do not follow the turn signs for SR 15. Continue straight past the historic Warsaw Cut Glass Company and follow the road around the corner. You will see Warsaw Meadows on the right. If arriving from the South, you will turn right onto Prairie street from SR 15 just before Marsh Supermarket. Warsaw Meadows will be located on the left.
