Attending the **P Transition to Residency Conference this weekend and I have been learning so much as I bring discussions regarding the initiation of more programs for to receive structured training and cultural education to help them mold themselves into American Healthcare.

Understanding signaling on a larger scale, comprehending how programs can help IMGs, and discussing big pain points has been priceless.

It's been great working with the aspirants this year and I personally vouched for many at various programs. Will do interview skills this coming weekend. And excited to work closely with aspirants starting July 2026!

Four patients who underwent robotically assisted aortic valve replacement (AVR) with transcervical access were able to resume an active life one week later without any restrictions, reporting low-level pain during recovery.

Cleveland Clinic cardiothoracic surgeon Marijan Koprivanac, MD, who performed the operations earlier this year, announced the results in a late-breaking presentation at the American Association for Thoracic Surgery annual meeting in May. The series is the first known report of the clinical application of transcervical robotic AVR.

“We are optimistic that this strategy could be a breakthrough for offering the benefits of surgical AVR without the lengthier and more painful recovery associated with current options,” says Dr. Koprivanac.

Robotic assistance for AVR is now in the vanguard of this evolution, conferring the advantages of smaller incisions, enhanced visualization and greater precision compared with other surgical methods. Combining robotics with a transcervical approach — potentially offering an even less-invasive option — is being explored by a few groups, but Dr. Koprivanac is unaware that it has been performed by others beyond feasibility studies in cadavers.

As the first week of PGY1 comes to an end, I feel drained and exhausted, but I still wake every morning at 5 am with the same energy! A lot of that energy is thanks to my priceless seniors and the supportive attending I've had the pleasure of rounding with at and I look forward to excelling in my time here.

Keep thriving residents. It's not going to be easy, and the challenges don't end when our shift does. We have family, personal life concerns, and much more to do.

This first weekend will hit different :)

Rx is switching up the pharmacy reimbursement game.

On March 20, Group-owned Optum, one of the Big 3 pharmacy benefit managers (PBMs) in the US, announced it would move to a cost-based reimbursement model to “provide pharmacies with better financial means to stock more medicines” and “alleviate drug shortages,” according to a company press release.

“Our plan is to align drug payment models for clients and pharmacies more closely to the costs that pharmacies pay for those drugs,” Angela Adler, SVP of networks and transformation at Optum, told Healthcare Brew.

Once-daily obicetrapib (NewAmsterdam Pharma)—an investigational, highly-selective cholesteryl ester transfer protein (CETP) inhibitor—safely lowers LDL-cholesterol levels on its own and in combination with ezetimibe in patients with heterozygous familial hypercholesterolemia (FH) or atherosclerotic cardiovascular disease (ASCVD) already on maximum tolerated doses of lipid-lowering therapy, according to new data from two trials.

Inequities don’t exist only in the quality of healthcare women receive but also in how that care is reimbursed, according to a group of experts speaking before Congress on April 29.

The group—which included ob-gyn experts from Harvard, Northwestern, and more—held a briefing to say systemic inequities within the healthcare reimbursement system lead to providers making more money for some procedures performed on male anatomy than those performed on female anatomy, and therefore, those procedures on male anatomy get prioritized.

“A lot of people say, ‘Oh, you’re doing as much work, but do you have as much training? Do you have as much expertise? Are your procedures as complicated?’ It’s very easy to think that women’s issues are not as complicated or that their surgeons are not as well trained, and that is not that case,” Jocelyn Fitzgerald, a urogynecologist at the University of Pittsburgh Medical Center, said during the briefing.
