Transitions Healthcare Washington(formerly known as Humbert Lane) is a Skilled Nursing and Rehabilitation Facility that has served the community for over 29 years. We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

Transitions Healthcare Washington PA is proud to support a cause that’s close to our hearts! 💙

Now through May 25th, we’re collecting beef jerky 🥩 and protein bars 💪 to send to our deployed troops overseas. They're simple comforts that mean so much during long days away from home.

This effort is especially meaningful to us because it supports a family who is near and dear to our Washington family 🤝 — a family that is always giving back and going above and beyond for our residents and activity department. Now, it’s our turn to give back to them! 🙌
