Transitions Healthcare Washington(formerly known as Humbert Lane) is a Skilled Nursing and Rehabilitation Facility that has served the community for over 29 years. We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

A special shoutout to Mrs. H, a devoted family member of one of our residents here at Transitions Healthcare Washington PA. She brings in donations every time she shops, and her kindness truly warms our hearts. 💖
