It takes a team to care for our patients and families. When you or your loved one chooses hospice, members of the team care for the whole person - physically, emotionally and spiritually - and those who elect to receive hospice care sooner report a higher quality of life.
Hospice care is provided in the patient's home or a facility. The care team may include:
π A nurse to provide pain and symptom management and educate families on how to provide the best care.
π A physician to develop the care plan and certify hospice eligibility.
π A social worker to provide education, assist with advance directives and find resources to support the whole family.
π A home health aide to provide assistance with personal cares and respite for the caregiver.
π A music therapist to alleviate anxiety and provide comfort.
π A spiritual counselor to provide spiritual guidance, regardless of belief system.
A volunteer to provide companionship to the patient and family.
