This is what care management looks like in action.

Christina, one of our incredible Care Managers here in Boston, recently accompanied her client, Phyllis, to a primary care appointment, where she advocated for a new medication to help prevent UTIs. She is now coordinating delivery and simplifying medication management for Phyllis and her care team.

Beyond the appointment, Christina continues to support Phyllis’s ability to age safely at home by helping declutter her space, coordinating a chair lift installation, and keeping family members across the country updated every step of the way.

This is the kind of hands on, compassionate support that makes all the difference. We are so proud of the work our Boston team is doing every day to support families.
