To help keep patients out of ERs and Urgent Cares for items that could be managed by primary care, we offer acute visits to bridge the gap while on wait lists.

We offer telehealth acute visits for patient's between primary care providers for items such as needing medication refills, sick visits or help managing conditions like your diabetes or high blood pressure, etc.

Nicole Ray, FNP-C is a board certified Family Nurse Practitioner who has a broad range of experience and specialized in hospital medicine for over 7 years.

Ms. Ray is experienced in internal medicine and family practice. She has received additional training in the management of substance use disorders and weight loss treatment. She is a clinical leader who has worked in management and has a passion for teaching. She continues to work full time in geriatric care while running her own small private practice and work as medical director of SUD clinic. She also continues to moonlight as a Hospitalist at serval local hospitals.

Ms. Ray graduated with her MSN-FNP from Saint Joseph’s College of Maine in 2017. She received board certification through the American Academy of Nurse Practitioners in 2017. Prior to becoming a FNP she spent several years as a charge nurse.

Nicole was born and raised in Central Maine. She is married with two grown children and has one grandson. She enjoys spending time outdoors with her family and dogs.
