Iroquois Memorial Hospital offers state-of-the-art diagnostics with a 160 slice CT, MRI, Nuclear Medicine, 3D/4D Ultrasound, and 3D Mammography. Family medicine, internal medicine, and pediatric services are provided through 4 satellite clinics in area communities. Additional services include a specialty clinic with visiting specialists for different specialties and a state-of-the-art surgery center. Home Health & Hospice Services, an attached nursing home and an outpatient rehab center offering PT, OT & Speech Therapy complete the medical services provided to the population in East Central Illinois and West Central Indiana.

State and federal laws restrict certain types of information that the parent of a minor child, age 12 and older, may access without the consent of the minor. We’d like to remind families that, in line with our established policy, portal access changes when a child turns 12. At that time, access is limited to the patient’s account (parent view), and full proxy access for parents or guardians requires a signed proxy authorization form. While this policy has been in place, we will now be applying it consistently across all accounts. If you would like to maintain proxy access, please complete a proxy authorization form by contacting our Health Information Management department at 815.432.7715.
