Alta Rehab at Wauconda Alta Rehab at Wauconda offers short-term rehabilitation, pain management and wound care. Visiting hours are from 10 am - 9 pm daily.

Alta Rehab at Wauconda, a CMD Rated 5 Star Facility, is a proud member of the community, has provided health care to area residents of all ages for over 25 years. We have a growing reputation for providing some of the area’s best post-hospital care and short-term subacute rehabilitation, which includes our innovative Cardiac Care Program and dynamic orthopedic rehabilitation services. We also offer one of the area's only designated Special Care Units, (as designated by the Illinois Department of Public Health). This dedicated unit provides a safe and caring environment for persons who are in need of Alzheimer and Dementia care. In addition to these services, we also provide traditional long-term care, respite, Hospice, and palliative care. Alta Rehab at Wauconda is also home of the CHOICE program, an award-winning program recognized by a leading post-acute care magazine. CHOICE, which stands for Choosing How Our Individualized Care Evolves, is a patient-centered program focusing on the needs of the individual. Alta Rehab at Wauconda is a small, comfortable facility where families and friends are always welcome. The uniquely designed facility includes a variety of spaces which promote independence, safety, community, and recovery. Such features include our outdoor garden and patio, Town Square, colorful bird aviary, and movie theatre. We welcome friends and family. Please come by for the day or for any special event.
