The mission of Life Navigators is to improve the quality of life for individuals with disabilities.

Our Services Include:

Information and Education
Provides a variety of services that impact individuals with disabilities and their families. Search our Online Resource Guide for information on over 300 community resources. Self-Advocacy and Independence
Organizes opportunities for kids and adults to participate in community outings, educational trainings and volunteer opportunities. Trust Program
Offers planning assistance for the financial and service needs of individuals with disabilities to create bright futures. Guardianship
Secures the health, safety and happiness of individuals with disabilities who may not have a family member to provide this essential monitoring and support. Family Support
Connects families of kids and adults with disabilities to community resources and programs. Care Management Unit
Coordinates long-term care services for individuals with disabilities.

Finding accessible outings is a challenge for Charde's group home. They discovered our events, and now, Charde participates in game nights, art classes, and karaoke. Without your support, Charde won’t have these opportunities that bring joy and connection into her life. Our monthly social events and educational trainings are offered at no cost since over 80% of those we serve fall below the County median income and are very low-income. We extend our gratitude to the parishioners of St. Elizabeth Ann Seton for once again running our Holiday Giving Tree this year. For 2025, they generously donated over 200 gifts for the consumers in our Guardianship and CLTS programs. Our staff is starting to hand these gifts out to their consumers, and we will post photos of holiday cheer soon! Thank you, parishioners of St. Elizabeth Ann Seton, for again being amazing supporters of individuals with disabilities in our community.

Our CLTS program is expanding! Our Children’s Long-Term Support Program started in September 2023 in partnership with Milwaukee County Department of Health and Human Services – Children, Youth and Family Services. Since September, the CLTS program has grown exponentially! Our experienced Program Manager and Support and Service Coordinators will help connect kids with disabilities and their families with the services they need to live a happy and healthy life.
