The Clinical Care Coordinator provides clinical and administrative services within the scope of practice standards in collaboration with registered nurses and other members of the health care team and teaches and educates patients and their families, as well as Personal Care Workers. All Clinical Care Coordinator services in the home is performed under the supervision of the Registered Nurse Manager.
