It is with very heavy hearts that we announce the permanent closure of the Ear, Nose & Throat Clinic of Coffee County, the Westside Surgery Center, and FYZICAL Therapy & Balance Centers – Douglas.
Our administrative offices will remain open through April 13, 2026, operating only from 312 Westside Drive in Douglas, GA and 2016 Pineview Avenue in Tifton, GA. After April 13th, all office locations will be closed; however, the email address admin@entclinicmd.com will continue to be monitored, and all emails will be answered. This will be the method by which patients may request their medical records from the medical record custodian.
This practice was founded in July 1996 by Dr. Jeffrey L. Silveira, MD, whose compassion, faith, and calling to serve others shaped every part of what this clinic became. Jeff and Hope began this journey in a small four-person office, and through dedication, prayer, and hard work, grew it into a thriving multi-location practice that eventually included three offices, FYZICAL Therapy & Balance Centers, and a full surgical center—employing more than 80 team members and serving patients across multiple counties.
Jeff had a heart for people and a gift for making every patient feel seen, safe, and loved. One of his greatest joys was giving small tokens of comfort to the patients he was taking to surgery—stuffed animals, toy cars, and most cherished of all, Bibles. These simple gifts reflected his deep compassion and devotion to caring for others.
Most importantly, Jeff prayed over each of his patients before surgery, believing wholeheartedly that it was God working through him in every procedure, every diagnosis, and every moment spent caring for this community. His unwavering faith, gentle spirit, and immense kindness touched countless lives and created a legacy that cannot be measured. His passing in June 2023 was a profound and heartbreaking loss to our staff, our patients, and the community he served with such devotion.
After his passing, Dr. George Livermore joined the practice and continued Jeff’s legacy by providing excellent ENT care to the patients who had depended on us for so many years. Unfortunately, Dr. Livermore resigned effective March 1, 2026, and due to this resignation, we are no longer able to remain open.
Dr. Silveira’s wife wishes to express her profound sadness over the closing of the practice her husband built, loved, and poured his heart into. For nearly 29 years, caring for this community has been one of our greatest blessings.
Continuity of Care and Patient Notifications
To support every patient during this transition, especially those with upcoming or recently scheduled appointments, we will be contacting patients through:
• Letters by mail
• Phone calls
• Secure text or email messages through Klara
All patients who had appointments scheduled between March 1, 2026 and April 13, 2026 will be contacted using one or more of these communication methods, with clear instructions for obtaining medical records and transitioning their care.
This process ensures that we meet all federal and state continuity-of-care requirements.

Requesting Your Medical Records
Patients may request their medical records by calling 912-384-2200 or emailing admin@entclinicmd.com through March 31, 2026.
Beginning April 1, 2026, all medical record requests must be submitted exclusively by email to admin@entclinicmd.com.
Phone, fax, mail, and in-person requests cannot be accepted after March 31.
After April 13, 2026, all office locations will be closed, but admin@entclinicmd.com will remain monitored and active to ensure all requests are answered by the medical records custodian.
