Euverita Health & Wellness Health & Wellness Clinic Medicare, Medicaid, and state-based public insurance options require an order from a qualified provider to receive reimbursement.

Testing Services are provided under a "fee-for-service", patient-directed testing model.

**We accept: Cash, check, and major Credit cards
-Payment is expected at the time of service. Failure to pay for services may delay the release of results.
-Health Savings Accounts, common with many High Deductible Health Plans, may be used; testing is considered a qualified medical expense.

-We do not accept or bill insurance; many private insurance providers may reimburse you for all or part of the cost of testing based on their agreement with you. Typically, this requires downloading a reimbursement claim form from your insurance provider's website, filling it out, and submitting the form to your insurer.

-We do not accept or bill Medicare, Medicaid, or state-based public insurance options. Patient-directed testing, which comprises our entire menu, do not qualify for submission to these insurance options. Patients who utilize these insurance options, including Medicare supplemental insurance options, will be responsible for the payment of any services ordered.
