Because deep down…
They already know they want something different.

They’re just stuck in the loop of:
“What if it doesn’t work?”
“What if I mess it up?”
“What if I’m not ready yet?”

But here’s what’s interesting…

I just interviewed a nurse with 26+ years of experience who felt the exact same way.

NURSES: Here’s the actual checklist for starting your IV therapy business (no fluff)

I see the same questions every week about what’s actually required to get compliant and start serving clients, so here’s the real breakdown:

The Non-Negotiables:

âś… Medical Director - They supervise your practice and sign off on what you can do under their license. (NPs in some states are exempt)
✅ MD Contract - Spells out exactly what you can and can’t do. If you’re an NP, you’ll need a prescriptive authority agreement too.
✅ MSO/MSA Contract - If you’re in a corporate practice of medicine state (like Texas), this is how you legally get paid. The money flows through the doctor. Non-CPOM states like Florida?

You’re good without this.

✅ Practice Insurance - Get at least $3M/$1M coverage and add your doctors as additional insured. This isn’t optional.
âś… Signed Protocols - Every IV, injection, GLP-1, peptide... ALL of it needs signed protocols from your medical director. Update them yearly.
âś… HIPAA-Compliant EMR - You need somewhere legit to store charting, consent forms, all of it.
âś… Good Faith Exams - Gone are the days of standing protocols. Get a service that does good faith exams for a fee. Your clients get cleared for a year (sometimes less for things like GLP-1s).

This is HUGE for compliance.

The Often-Overlooked Stuff:

🔍 Call your health department and city - requirements vary wildly. Mobile or brick-and-mortar, just call and ask what they need.
🔍 State-specific rules - Some states don’t let RNs compound/mix. Some require a doctor or NP on-site or within a certain distance. Some have telehealth restrictions. KNOW YOUR STATE.
🔍 If you’re doing aesthetics - example: in Texas, your medical director needs to be registered with TDLR. In Florida, they typically need to be a derm or plastics doctor with distance requirements.

If You’re Going Mobile:

You’ll need the basics: IV poles, stools, BP cuffs, an iPad (way better than a phone for client charting), mini fridge at home for cold storage (separate from food), lockable storage for supplies, and a solid case to transport everything safely.

Bottom Line:

The medical compliance side isn’t that compplicated once you know what to look for. The confusing part is that every state has its own quirks.

My advice? Start with the non-negotiables, then dig into YOUR state’s specific requirements before you spend a dime.
