We are proud to share that our Hospice Journey Project Team has been named a 2026 Trailblazer for Quality and Innovation by HCANYS — the Home Care Association of New York State.

The award recognizes the team's months-long initiative that significantly increased the percentage of hospice patients who receive in-person visits from a registered nurse or medical social worker during their final three days of life, a key quality measure tracked by the Centers for Medicare & Medicaid Services (CMS).

We couldn't be prouder of the nurses, social workers, and leaders who made this possible. 💙
