Our story starts with Sally Dondes. If you were to walk into Sally’s apartment years ago, it would be obvious why she would not want to leave. An avid antique collector and a pillar in the community, her home was filled with treasures from her busy life. Sally was daughter of immigrants and grew up during the great depression. Her life experiences taught her about independence. They also taught her the value of community and honed her skills as a successful businesswoman. For most of her adult life, she was a landlord managing her and her husband’s properties. Sally was well known on Church Street by her routine and the table she set up at what was then known as Woolworth’s, which is now Outdoor Gear. She had great stories about helping to shape Burlington, like how she was the first customer of the Vermont Teddy Bear Company by buying a teddy bear from the cart which became the legendary company it is today. When Sally fell, she needed help getting through rehab. She did not qualify for home health care and was unable to live in her apartment alone. She did not want to leave her home. Luckily, she knew an outstanding young man named Mohamed, who was going through nursing school at the time. Mohamed had often helped Sally during weekend trips out to the lake over the summers and was familiar with her routines. He helped her regain her independence by getting her out of rehab a full four months faster than expected. It did not take long to realize that Sally was not alone. Vermont has a rapidly aging population and it has become alarmingly evident that not only is in-home care urgently in demand but assisted living facilities throughout our state and in New Hampshire are in desperate need of qualified caregivers. The experience with Sally taught Mohamed what experts in elderly care already know; That the best care is compassionate care at home. Our parents, grandparents, and loved ones deserve to live their best lives possible. They Deserve The Best Care Possible. With this realization, TLC HomeCare was born. TLC is committed to providing the best care possible for your loved ones. TLC is the only skills tested and trained agency with nurse evaluators to oversee individual client cases and provide ongoing training and support groups to our caregivers and members of our community including law enforcement and first responders. No other agency in Vermont provides 24/7 access to an on-call nurse, nursing follow-ups every 30, 60 and 90 days and PACT training to our staff and local population through our very own TLC Academy. Anyone can start home care in Vermont, but not all have the proper skills or provides the level of care TLC professionally trains for. Job Responsibilities:

Caregivers and LNAs are expected to perform as outlined in a written care plan approved by a Nurse Case Manager. The Care Plan is available to caregivers in the client's home and online through our scheduling program. All Caregivers and LNAs are supervised by a nurse case manager. All Caregivers and LNAs receive scheduling direction from the HomeCare Coordinator.

The duties of a Caregiver or LNA may include but are not limited to the following:

You will be working with our elderly clients helping them with day to day activities so that they can age in place.
You will have a MAJOR impact on their life…helping them with things like
Bathing and dressing so they can start their day feeling good

Preparing nutritious meals for them so their body can heal and so that they can have the energy they need to get through the day.
Making sure they are taking their medications.
Sometimes running errands with them…
Helping with some light housekeeping so that their environment is safe….

Mobility Aids for your Loved Ones

As we all get older, it gets harder and harder to move, but most are likely to retain their independence until quite late in life. Canes for Walking

Among the most common devices purchased are canes. There are several sizes, designs, and colors available to your elderly loved one. The prices vary greatly, but some of them are within your budget. When canes are used correctly, they can help prevent falls for the elderly. They can help improve balance and stability as well. The duties of a Caregiver or LNA may include but are not limited to the following:

• Planning and preparing meals
according to specific dietary requirements of the client.
• Maintaining a safe, clean, and healthy environment through light
housekeeping; for example,
changing bed linens, dusting and vacuuming, cleaning kitchen and
bathroom, and laundry.
• Providing companionship and recreation for the client including
reading, walks, etc.
• Accompanying the client to doctors or other appointments.
• Grocery shopping or other errands with or without the client as
needed.
• Performing other delegated tasks as indicated in the Care Plan.
• Perform any personal care for the client; for example, toileting, hair
care, oral hygiene, bathing, and grooming.
• Assist the client with position transfers and other types of mobility
as directed by the Care Plan.
• Monitor the client’s self-administration of medications and/or use
of medical devices as specified in the Care Plan
