We are a physician-led organization dedicated to building a better community-based health care system centered around the relationship between you and your Primary Care Physician. Tandigm believes PCPs are in the best position to help navigate an increasingly complex and expensive health care system. We provide PCPs with innovative clinical and technologic support so they can take the best care of their patients. Health is on the way!

More evidence of the value of strong primary care: a recent study found that older adults with a regular source of primary care may live longer. Adults ages 65-84 with regular access to a primary care provider had a lower 15-year mortality risk, and a median survival nearly two years longer than those without consistent care. Last week Tandigm held our annual Holiday Raffle Luncheon. Thanks to the incredible generosity of our teammates, we raised $9,668.00. Props to our Activity and AWARE teams for a great and giving-focused event!

At Tandigm, teamwork drives patient outcomes. When a P*P noted a patient in need during a Tandigm office visit, our teammate looped in the Nurse Care Manager. She quickly connected with the patient to educate, support and add them to our Heart Failure Pathway.
