CLIA regulations require a facility to be appropriately certified for each test performed. To make sure Medicare & Medicaid only pay for laboratory tests performed in certified facilities, Medicare edits each claim for HCPCS code considered a CLIA laboratory test at the CLIA certificate level. CMS tells the MACs about the new HCPCS codes that are both subject to CLIA edits and excluded from CLIA. MACs will deny payment for claim with the HCPCS codes mentioned as subject to CLIA edits to a provider: *Without a valid current CLIA certificate. *With a CLIA certificate for waiver (type 2)(when billed without the QW modifier). *With a CLIA certificate or provider performed microscopy procedures (type 4) (when billed without the QW modifier)
Helpful information: MACs won’t research their files either to retract payment for claims already paid or to retroactively ay claims. However, they will adjust claims that you bring to their attention.
