The posterior longitudinal ligament runs along the back of the vertebral bodies inside the spinal canal. In OPLL, this ligament gradually hardens into bone. As it thickens, it can narrow the spinal canal and compress the spinal cord.
Because OPLL can progress slowly, symptoms are sometimes subtle at first. But once the spinal cord is compressed, early evaluation is critical.
